This form allows applicants to request a login for the Ofwat customer self-service portal. This portal is used to submit applications for Water Supply and/or Sewerage Licences and New Appointments and Variations.

Please note, the portal allows for one login per organisation and the login can be used by mulitple colleagues by using the same login and password. We would recommend you use a generic email address for your company as your group shared login.
